What is Vitrectomy?

The clear, gel-like fluid filling the inside of the eye is called vitreous. Various diseases can cause the vitreous gel to become filled with blood or to cloud and harden. This condition disrupts the proper passage of light entering the eye, preventing it from reaching the retina accurately.

Vitrectomy is a surgical procedure aimed at removing the vitreous from the central cavity of the eye to correct vision. This surgery has demonstrated its effectiveness in treating various conditions such as Retinal Detachments, Vitreous Hemorrhage, Macular Holes, Diabetic Retinopathy, and Macular Pucker. One significant advantage of Vitrectomy surgery is its ability to stabilize and enhance vision.

Eye Disorders Treated by Vitrectomy

The eye conditions that need a Vitrectomy include :

  • Complications of Diabetic Retinopathy like bleeding or retinal detachment.

Procedure for Vitrectomy

The Vitrectomy surgical procedure is conducted on an outpatient basis, under either general or local anesthesia, and typically lasts 2-4 hours. Tiny incisions are made within the eye to introduce microscopic surgical tools, including a suction device, scalpels, and an internal light source. The vitreous is replaced with either an intraocular gas compound, silicone oil, or a saline-based solution to maintain proper eye pressure and stabilize the natural vitreous tissue. The surgeon closes the incision site using small absorbable sutures. Additionally, antibiotic drops or injections may be administered to prevent infection. In some cases, Vitrectomy can be combined with Retinal Detachment Surgery or Laser Eye Therapy. The recovery period generally spans 2-4 weeks.

Vitrectomy Technique

A surgeon may employ various techniques for vitrectomy surgery, including :

Silicone Oil : Silicone oil is utilized instead of gas to maintain the attachment of the retina after surgery. This technique proves beneficial when a more prolonged support for the retina is required.

Scleral Buckling : This technique is employed in conjunction with vitrectomy to provide additional support to the reattached retina.

Intraocular Gases : Either Sulfur Hexafluoride gas or Perfluoropropane gas, combined with sterile air, is employed. This combination remains in the eye for extended periods, proving effective in flattening and maintaining the attachment of a detached retina during the healing process. Gas injections are also utilized for closing macular holes.

Endoscopy : This technique aids in visualizing the intraocular structures by projecting the inside of the eye onto a monitor. It proves particularly valuable when the lens or cornea is cloudy, obstructing a clear view under the microscope.

Lensectomy : The technique is employed to extract the crystalline lens of the eye during vitrectomy surgery.

Microsurgical Instruments : Forceps, picks, and scissors are the three microsurgical instruments employed for manipulating intraocular structures during surgery.
